Child Health Care Plans

Introduction

There is statutory guidance in place to ensure:

  • Pupils at school with medical conditions should be properly supported so that they have full access to education, including school trips and physical education.
  • Governing bodies must ensure that arrangements are in place in schools to support pupils at school with medical conditions.
  • Governing bodies should ensure that school leaders consult health and social care professionals, pupils and parents to ensure that the needs of children with medical conditions are properly understood and effectively supported.

Health Care Plans are a vital part of the school meeting the medical needs of individual children and the statutory obligations the school is accountable for.

Online Health Care Form

Once this form has been completed the medical lead or a member of the medical team will contact you to arrange an appointment (face to face or telephone) to finalise the health care plan. For your records, you are able to download a copy of your submission once you have submitted. You should typically expect contact from the school team within three working days of submission - if you have not had contact from the school by the end of the third day please email office@chesswood.w-sussex.sch.uk.

If there are significant changes to an existing plan you will need to complete  a new form.
